A transition plan must be developed for all children who are about to leave the Early Intervention Program.

The plan will outline the steps needed to help your child in the preschool setting, the steps to prepare service providers help your child in the transition, and will list the people who are responsible to help you during the transition process.

If your child needs preschool special education, the Early Intervention Official will ask for your permission to contact your school district's Committee on Preschool Education (CPSE).

At least 90 days before your child's third birthday, the Early Intervention Official can schedule a conference with you, your service coordinator, and the chair of the CPSE in your school district to discuss your child's transition.

The CPSE will evaluate your child to determine if she continues to need special services.  If your child has a disability that may be affecting his or her learning, the CPSE will find your child to be an eligible "preschool student with a disability." The CPSE will also recommend the program or services to meet your child's individual needs, and where they will be provided.

If your child is an eligible preschool student with a disability, you and the other CPSE members will write an Individualized Education Program (IEP) for your child that will list the recommended services to be provided, how often, and for how long.  The recommendations will be forwarded to your local school district Board of Education for approval.
